Lifetime Care and Support Scheme
Occupational Therapy Network provides case management, occupational therapy, FIM™ assessment and care needs review within the NSW Lifetime Care and Support (LTCS) Scheme. OT Network intervention is person-centred, focusing on the participant’s specific needs and providing services within the Scheme.
Our case managers coordinate treatment, rehabilitation and care requirements to maximise independence and participation. The participant is central to decision-making. Plans are developed in collaboration with the participant, family/carer, service providers and the Lifetime Care and Support Authority. Approved services are provided in a timely manner. With expertise in case management, occupational therapy and severe injury we offer innovative strategies tailored to the individual participant’s successful reintegration into the community.
Our occupational therapists have a wealth of experience in traumatic brain injury, spinal cord injury, amputation and multiple injuries.
Occupational therapy is provided to facilitate function and independence across a range of areas including activities of daily living, aids and equipment, upper limb splinting, cognitive retraining, home modifications, attendant care training, community access and vocational rehabilitation.
Occupational Therapy Network intervention is evidence-based and cost effective to achieve sustainable outcomes.
