Lifetime Care & Support

Occupational Therapy Network provides case management and rehabilitation for people eligible for the NSW Lifetime Care and Support (LTCS) Scheme.

Our case managers coordinate treatment, rehabilitation and care requirements for people with severe injuries to address the individual’s specific requirements. This includes assessment, planning and goal-setting, involving the participant in decision-making about meeting their needs.

OT Network case management and rehabilitation focuses on achieving maximising participation and independence, resumption of function and fulfilment of life roles. At OT Network, we understand that the best results are achieved when the involved parties communicate effectively. Our case managers work in partnership with the participant, carers, family, service providers and the Lifetime Care and Support Authority to achieve agreed goals.

OT Network assessments focus on function and ability, identifying limitations and strategies to achieve goals. Assessments address areas such as cognitive, behavioural, psychological and physical impairment taking into account quality of life issues such as physical and emotional wellbeing.

We strive to achieve sustainable outcomes within the framework of the Lifetime Care and Support Scheme. The Participant’s Lifetime Care & Support Plan, Community Discharge and Community Living Plans clearly outline the participant’s requirements and support needs as well as charting progress and achievement of goals. Care needs are assessed, monitored and reviewed to ensure an appropriate level of support is provided throughout the various stages of life and transition in life roles.

OT Network case managers are also occupational therapists and are able to provide assistance with the prescription of aids and/or equipment to facilitate function as well as home and/or vehicle modifications to enhance the participant’s independence.

With experience in severe injury and a range of disabilities as well as case management, we are able to offer creative options tailored to the individual participant to achieve successful reintegration into the community.
